Governor Babajide Sanwo‑Olu announced on Tuesday that all Lagos residents must obtain health insurance by 31 December 2026, with the compulsory scheme taking effect on 1 January 2027. The deadline was communicated through a statement from his Special Adviser on Media and Publicity, Gboyega Akosile, and was delivered at an event in Victoria Island attended by officials, health practitioners and private‑sector stakeholders.

This declaration came at the same time as the introduction of the Lagos Private Health Plans (LPHP), an innovative program designed to provide insurance for employees with fixed incomes and business entities. The LPHP will function in parallel with Ilera Eko, the current framework that serves disadvantaged citizens and those employed in the informal economy. According to the statement, 21 health‑maintenance organisations have pledged to participate in the private plans, creating a partnership between the state and private providers.

Background

The mandatory health‑insurance policy was introduced under Executive Order EO/BOS/01 of 2024, which obliges residents, employers and workers to secure coverage. The order seeks to widen access to healthcare and lessen the financial strain of medical bills on households. Earlier, the state had planned to begin full enforcement in the third quarter of 2025, but the deadline was later extended to the end of 2026. The Health Equity Fund was also increased from N1 billion to N3 billion to subsidise insurance for more than 500,000 vulnerable residents who cannot afford premiums. Commissioner for Health Prof Akin Abayomi highlighted that advances in medical technology have raised delivery costs, reinforcing the need for an inclusive insurance system.

Why it matters

The policy affects every Lagos resident, as well as employers and workers, by making health insurance a legal requirement. By stabilising patients before addressing payment or insurance issues, the government aims to prevent financial barriers from denying emergency care. The expanded funding and private‑plan collaboration are intended to protect households from out‑of‑pocket expenses and to support healthcare facilities facing payment delays. Employers are urged to treat healthcare contributions as essential business costs, comparable to pension obligations, thereby influencing corporate budgeting and employee benefits.

What happens next

Residents have until the end of December 2026 to enrol in either the existing Ilera Eko scheme or the newly introduced private health plans. Health‑maintenance organisations are expected to finalise their participation agreements in the coming weeks. The state will begin enforcing the mandatory coverage on 1 January 2027, at which point non‑compliance could trigger penalties as outlined in the executive order. The government has also indicated that emergency treatment must be provided irrespective of a patient’s insurance status, reinforcing the policy’s emphasis on immediate care.
